Information regarding the completion of the ACCESS Community School Referral Form
Completion of this referral form does not guarantee enrollment in ACCESS. Enrollment is subject to space and program availability. Once the completed referral form has been reviewed and approved by ACCESS, both the school district and the parent/guardian/caregiver/adult student will be notified by ACCESS. REFERRING DISTRICT REFERRAL
For Referring
Districts:All supporting documents required to be submitted with this referral form must be uploaded to the ACCESS Referral secure file portal, which includes documents listed below for General Education and Special Education Students:
Documents

- Immunization Record
- Attendance Certificate
- Current Transcripts (secondary students)
- Withdrawal Grades
- Discipline Record
Additional Documents
Required for
Student with an IEP
- Authorization of Release of Information signed by parent(s) allowing information to be shared
- Copy of the last signed and implemented Plan Review/Annual IEP (Must be current and not due within 30 school days of hypothetical first day of instruction with ACCESS)
- Eligibility Evaluation/Triennial (Must be current and not due within 90 calendar days of hypothetical first day of instruction with ACCESS)
- If applicable, a Copy of Speech and Language Assessment Report, Mental Health Assessment Report, Behavior Intervention Plan
Students with IEPs: For students with an Individualized Education Plan (IEP), the school district must attach the required Special Education documents listed above in addition to the documents required of all students. ACCESS Special Education will consult with district representatives to ensure students are able to receive FAPE in ACCESS. If a student is in the process of being assessed for Special Education, please refer after your assessment and IEP is completed to discuss if ACCESS would be an appropriate FAPE. **

We recognize that there may be situations where districts are unable to complete the online referral form. For students in the following categories, a print version referral form is available:
- Currently in ACCESS
- Currently Incarcerated
- Foster Youth
- Unaccompanied Minor
