ACCESS Referrals

Information regarding the completion of the ACCESS Community School Referral Form 

This referral form is to be completed by a school district administrator/designee. Please fill out this form completely. 

For students with an individualized education plan​ (IEP) the school district must complete the ACCESS Special Education Referral Process PRIOR to submission of this referral form. To initiate​ the ACCESS Special Education Referral Process, please contact at accessspedrecords@ocde.us​​ or call (714) 547-7931. The ACCESS Community School referral form is not considered complete until the ACCESS Special Education Referral Process has been completed for a student with an IEP. 

Completion of this referral form does not guarantee enrollment in ACCESS. Enrollment is subject to space and program availability. Once the completed referral form has been reviewed and approved by ACCESS, both the school district and the parent/guardian/caregiver/adult student will be notified by ACCESS. REFERR​ING DISTRICT REFERRAL

For Referring Districts: All supporting documents required to be submitted with this referral form must be uploaded to the ACCESS Referral secure file portal, which includes:

    • Immunization Record
    • Attendance Certificate
    • Current Transcripts (secondary students)
    • Withdrawal Grades
    • Discipline Record​

​After the referral form has been submitted, a copy of the form will be sent electronically to both the parent/guardian/caregiver/adult student and the school district representative for electronic signature via DocuSign. Upon receipt of all signatures, the referral form will be reviewed by OCDE.

Please note: Electronic signatures are required. The ACCESS Community School Referral form will not be reviewed until all required signatures have been electronically submitted. ​

EXCEPTIONS

We recognize that there may be situations where districts are unable to complete the online referral form. For students in the following categories, a print version referral form is available:

    • Currently in ACCESS
    • Currently Incarcerated
    • Foster Youth
    • Unaccompanied Minor
