Complete the Curriculum Fidelity Log after you have administered the curriculum.

2023-2024 TUPE Activities Submission Form
Complete the TUPE Activities Form when you have completed a campus-wide prevention activity, a youth development project, or family event.

Student Surveys:

*Administer Curriculum Survey to students upon completion of the curriculum.

**Administer Youth Development Project Survey upon completion of the youth development project. Only survey students involved in the planning and implementation of the youth development project.
